Please fill in this form and then click on the button at the bottom of the page marked 'Click here to print form'. The information that you have input will be dropped into a copy of the agreement and two copies will be printed. Keep the one entitled 'Customer Copy' for your records. The other copy must be signed and posted to us (along with two forms of identification which show your current address - driving licence, utility bill etc.). Please do NOT write in the start of the Standing Order as these will not be accepted by the bank. We are happy to accept photocopies. Please do not send us originals of sensitive documents like birth certificates etc. PASSPORTS ARE NOT ACCEPTED.
